This is a great list but you've left out two very strong and fairly inexpensive packages: Snap Survey, by Mercator and Remark and Web Surveyor, both by Gravic. Both allow the creation of scanable paper surveys and web based surveys. Snap, a British import, also has a provision for PDA based surveys. Also omitted is Survey Monkey which is similar to Zoomerang.

This is a good review of the Survey tools available.

For a simple satisfaction questionnaire I'd recommend Survey Monkey as another alternative.

I've been using it for years and in my mind is one of the best values available for those needing a basic tool.
